Charges for your sedation care are separate from your dental charges. Payment may be made in cash, cashier check, debit or by credit card and are due the day of service. Cashier checks may be made out to Blue Sky Anesthesia Associates. No personal checks will be accepted.
A deposit of $850.00 is required before sedation will be scheduled, and will then be applied to your final charges. The deposit is nonrefundable if canceled less than 48 hours prior to the procedure time. If your procedure is rescheduled due to illness or fever by your anesthesia provider or your dentist the deposit will be applied to the rescheduled date.
Your receipt will have insurance billing codes to assist you in collecting reimbursement from your insurance company should that be possible and Blue Sky Anesthesia Associates does not guarantee any reimbursement from your insurance company.
